2–3 days for the historic core is enough to see the must-dos without rushing. Add a day or two if you want 4 days to add a slower pace and day trips and time to explore at a slower pace.
Many of Prague's major sights sit close enough to reach on foot, and the metro covers the rest for longer hops — Travinora's itineraries route between stops using real transit times either way, not a walking-only assumption.
Apr–May, Sep–Oct tends to have the best weather-to-crowd balance — outside those windows you'll still find real bookable activities (1945 real options are searchable here right now), just with different weather and crowd levels.
If you're moving between neighborhoods more than once or twice a day, a short-stay transit pass on the metro usually pays for itself faster than single tickets.
It can be, but real activity pricing here averages around $283 per person per day — plenty of Prague experiences fit a range of budgets.
